Please read this section carefully. It is designed to help prepare you for surgery. Many of your questions are answered in this section.
In order to qualify for surgery, you will need to complete nutrition and psychological evaluations and certain diagnostic tests. This prescription is given to you at your first appointment. You will not be scheduled for surgery until all your tests and your nutrition and psychological evaluations have been forwarded to the center.
Please be certain you obtain all necessary referrals that may be required from your insurance company. If you are not sure if you need a referral, check with your primary care physician, otherwise you may not be reimbursed for the tests.
Depending on your insurance, you will need to schedule each of your tests with the corresponding department in the hospital or your primary care physician. Many of our patients like to schedule their tests for the same day to save time. We have provided a Patient Worksheet to assist you with your appointment dates and times.
Please call the Center to schedule an office visit when all your tests are completed. This must be done at least two weeks prior to your surgery date. At this time you will sign the necessary consent forms for surgery. Please bring a family member, friend, or significant other with you at this time.
For all tests, please report to the Outpatient Registration Department first. Outpatient Registration is located on the main floor of the hospital. Please bring your prescriptions and any referral forms required.
