Pre-Surgery Checklist

You must…

• Complete all the pre-operative tests ordered by the physician including nutrition and psychological evaluations.

• Two weeks prior to your surgery, you must call to make an appointment for Pre-Admission testing (PAT).

• Make an appointment with our Center Surgeons to sign your “Informed Consent” 201-227-5533.

• Review your “Shopping List” and obtain the necessary vitamins, minerals and protein shakes.
